    Driving Record Abstract (Three-Year)

    A driving record abstract is a three-year record of all moving violation convictions, accident involvement reports and other actions that result in license suspensions, revocations or other disqualifications. This record only displays the most recent driver license issuance date.

    Driver Record for Employer of a CDL Holder

    Employers of a CDL holder may request a driver record which includes all federally-required driver history and medical examiner certification information, if available. How to complete the BMV 1173.

    Unofficial Copy of Your Driver Record (Two-Year)

    The Ohio Bureau of Motor Vehicles (BMV) permits individuals to view their moving violation convictions, accident involvement reports and other actions that result in license suspensions, revocations or other disqualifications for the last two years. The driver record is consistent with the Ohio point assessment and suspension law.

    Driving Record History

    A driving record history is a record of all moving violation convictions, accident involvement reports and other actions that result in license suspensions, revocations or other disqualifications which are maintained in the Ohio BMV database. This record only displays the most recent driver license issuance date.

    By mailing a completed Record Request (form BMV 1173) and a $5.00 fee to:

    Driver License History

    A driver license history record provides issuance dates for current and previously-obtained driver licenses and/or state identification cards.

    How to Request

    By mailing a completed Record Request (form BMV 1173) and a $5.00 fee to:

    Last Known Address

    The BMV is able to provide the last known address on file by comparing information from the driver, title and vehicle registration database.

    Vehicle Registration Record

    A vehicle registration record provides the most recent registration information related to the identifier provided. Vehicle registration records do not provide historical information.

    Vehicle Title Record

    A vehicle title record provides the most recent title information related to the identifier provided. If you qualify, title record may provide limited historical information.

    Vehicle Title Record
    Historical Chain of Ownership

    A vehicle title record chain provides list of vehicles titled to an individual or list of owners of a vehicle to include issuance dates.

    Establish a BMV Record Account

    The Ohio Bureau of Motor Vehicles (BMV) has four types of record accounts, which require different identifiers and return different types of records. Unique account numbers are assigned for each type of account.

    • Driver Abstracts
    • Vehicle Registration Abstracts
    • Title Abstracts
    • Last Known Address

    To establish an account, submit a written request on your company’s letterhead indicating:

    • The nature of your business
    • The category your business falls under (corporation, partnership or sole proprietor)
    • Type of account(s) you would like to establish
    • A detailed description of how your company will use the records
    • A detailed description of the Driver Privacy Protection Act (DPPA) exception that qualifies your company to receive records

    You may mail your written request to:

    After an Ohio BMV Record Account is Established

    Once the account is created and an account number has been provided, a minimum $200.00 initial balance is required. Each account requires a separate check payable to: Ohio Treasurer of State.

    • You MUST include the account number and the words, “BMV Record Account,” on all correspondence and/or checks. Any correspondence and/or checks received without this information will be returned.
    • Fees for each transaction are deducted from your account balance as each inquiry is processed.
    • You will be notified by mail when your account balance reaches $100.00 (or an amount designated).
    • Requests will continue to be processed until your account balance is depleted. Once your account has been depleted, no requests will be processed until the account is replenished.

    Please Note: the BMV record account service does not permit users to access or perform transactions online.

    Driver Abstract Account

    A driver abstract is a three-year record of all moving violation convictions, accident involvement reports and other actions that result in license suspensions, revocations or other disqualifications.

    • The cost per record is $5.00.
    • Requests may be submitted by email or mail.

    A driver abstract is processed by submitting one of the following identifier combinations:

    • Name and Social Security number
    • Name and Ohio driver license number
    • Name and date of birth

    Driver Abstract Information

    The driver abstract includes:

    • Driver license record (name, address, physical description, etc.)
    • Moving violations
    • Convictions
    • Suspensions
    • Revocations
    • Accident report listings
    • Other requested information pertinent to the license status

    Vehicle Registration Abstract Account

    A vehicle registration abstract provides the most recent registration information related to the identifier provided. Vehicle registration abstracts do not provide historical information.

    • The cost per record is $5.00.
    • Requests may be submitted by email or mail.

    A vehicle registration abstract is processed by submitting one of the following identifiers:

    • Vehicle plate number
    • Vehicle identification number (VIN)
    • Social Security number

    Vehicle Registration Abstract Information

    The vehicle registration abstract includes:

    • Vehicle owner(s) information (name and address)
    • Vehicle information (VIN, title number, plate number, make, model, etc.)

    Title Abstract Account

    A title abstract provides the most recent title information related to the identifier provided. If you qualify, title abstracts may provide limited historical information.

    • The cost per record is $5.00.
    • Requests may be submitted by email or mail.

    A title abstract is processed by submitting one of the following identifiers:

    • Vehicle identification number (VIN)
    • Title number
    • Social Security number
    • Name

    Title Abstract Information

    The title abstract includes:

    Detail Description
    • Vehicle owner(s) information (name and address)
    • Previous owner information, if applicable (name and address)
    • Vehicle information (VIN, make, model, year, etc.)
    • Lien holder information, if applicable (name and address)
    • Vehicle title number and title status
    Chain History
    • Vehicle owner information (name and address)
    • Vehicle information (VIN, make, model, year, etc.)
    • Vehicle title number and title status

    Last Known Address Account

    The BMV is able to provide the last known address on file by comparing information from the driver, title and vehicle registration databases.

    • The cost per record is $5.00.
    • Requests may be submitted by email or mail.

    A last known address record is processed by submitting one of the following identifiers:

    • Vehicle identification number (VIN)
    • Vehicle plate number
    • Title number
    • Social Security number
    • Ohio driver license number

    Last Known Address Information

    The last known address record includes:

    • Vehicle owner(s) information (name and most recent address filed with the Ohio BMV)
    • Date the information was provided to the BMV

    Your Personal Information
    Driver Privacy Protection Act (DPPA)

    The Ohio Bureau of Motor Vehicles (BMV) does not release personal information from an individual’s driving record to bulk mail distributors for surveys, marketing or solicitation.

    Protected personal information includes the person’s name, Ohio driver license number, date of birth and address (other than the five-digit zip code).

    With the "opt-in" policy, the BMV may release personal information to another party if we have written consent from you or if the other party has an applicable reason under the DPPA to receive the information.

    Unless applicable by law, a driver’s photograph, Social Security number, telephone number and medical or disability information are not released.

    Protection of Personal Information

    To protect your information, the BMV requires:

    • All individuals and entities requesting records that contain personal information must complete the BMV Record Request (form BMV 1173).
    • If the individual or entity has your consent, he or she must complete the Notarized Written Consent Release Personal Information (form BMV 5008).
    • If the individual or entity meets one or more DPPA exemption, he or she must state which exemption and provide a verifiable identifier or relevant documentation associated with the exemption.
    • If an entity requests records in bulk, he or she must enter into a contract with the BMV. If the information is not being used correctly, the BMV may suspend or cancel the contract and stop providing information.
    • The BMV does not release Social Security numbers, except to other government agencies and when the Social Security number is provided with the request.
    • The BMV does not release resident address information about peace officers, except in accordance with the law.

    Public Records

    ‘Public record’ means records kept by any public office. An example of a public record provided by the BMV is your driving record, which includes convictions, suspensions and accidents. However, your name, address (other than the five-digit zip code), date of birth and other personal information are redacted and not provided.
